Are you a Carer ?

What is a carer? A carer is someone who provides regular unpaid help and support to another person (It does not include those who volunteer as part of a group or organisation).

This might be helping someone with paperwork and financial matters, housework, shopping, assisting with medication or other personal care needs such as keeping clean and fresh, preparing, and eating meals or getting out and about.

Carers can be any age, including children and young people, and from any background. Being a carer is very rewarding, but it can be hard work as well.

It is vital as a carer you get the practical, emotional, and financial support necessary to ensure you can continue in the carer role.

Here at Great North Medical Group, we wish to support our registered patients who are carers.

In line with the government requirements the practice keeps a register of all Patients who are carers. This is to ensure we can offer you the right support at the right time.

If you are a carer, please let the reception team or clinician know so it can be recorded. If you are unsure, please complete the following form and give to reception  GPs A5 new .

 

YOUNG CARERS

There are at LEAST 720 young carers in Doncaster and 1 in 20 of those young carers miss school because of their caring responsibilities.

If you are a registered patient and a young carer, please let the reception team know.

 

PARENT CARERS

Parent carers provide support to their children, including grown up children who could not manage without their help. The Child/adult can be ill, disabled or have mental health or substances misuse problems. The term family carers is used to describe carers where the child is over 18.

Our Include service helps young carers, families and those who work with young carers. We want you to be healthy, happy, and focused on the future. Doncaster Parents Voice

Making Spaces

Doncaster Culture and Leisure Trust

Doncaster Culture and Leisure Trust provide access to a wide range of health and wellbeing facilities to enable all residents of Doncaster to maintain a healthy body and mind. BeWell

 

Be Well Doncaster is a free health and wellbeing service available across Doncaster to support you on your wellbeing journey.

Our fully trained health coaches can work with you on a one-to-one basis to improve your quality of life by setting small but manageable goals. Areas our coaches can support includes, but is not limited to:

Managing a long-term health condition

Healthy eating

Sleep issues

Low mood

Debt support

Stress and anxiety
